Registrar Forms

Forms from the registrar can be completed by logging into your MyGNTC account and clicking on the Student Services & Financial Aid tab.  Then select the necessary form from the Registrar Forms box. Completed forms are automatically sent to the Registrar’s Office for processing. Please allow 1-2 business days.

  • Address and Phone Number Change 
  • Application for Graduation 
  • Change of Major 
  • Enrollment Verification 
  • Name, Gender, or SSN Change 
  • Permission to Release Education Records 
  • Transient Permission Request
