ID Card Request Form

For Students, Faculty, and Staff

All Georgia Northwestern Technical College Students, Faculty, and Staff, must always carry their identification (ID) badges while on campus. Your GNTC ID card is your key to the many services and activities on and off campus.

Once you complete this form, you will be emailed/sent a text message when your physical ID card is ready for pickup. Please wait for the notification that your ID is ready before coming to campus to pick it up. Mobile IDs are texted directly to your phone and then you can save it to your phone's Wallet (Apple Wallet, Google Wallet, etc.). 

Photo Tips:

  1. Ask someone to take a photo of you against a solid-colored background. 
  2. Your photo should be of your head and shoulders only. 
  3. Take the picture with the camera or cell phone turned vertically, so as to eliminate extra space in the frame. 
  4. Upload your (jpg, png, or gif) photo to this form.
